About the Book

After a short introduction, the prologue begins with the plane crash of Delta Air Lines Flight 191 at Dallas-Fort Worth International Airport on August 2, 1985, which Scott witnessed while conducting a walk-around inspection of a plane he was about to fly to New York. The impact of this tragedy and the insight it provided propelled him on a professional journey assisting high-consequence organizations around the globe. For the first time, Scott has collected strategies for success that can help any individual, business, or organization achieve sustainable high reliability. It's not always easy, but it's within your grasp.

We see and understand the world through the lens of our experiences. But sometimes we learn the wrong lessons from our successful outcomes. Our risky systems and behaviors produce dividends until they don't, and we pay for our miscalculations with our fortunes, our lives, and sometimes the lives of others.

We live in a dangerous world. Many of the things we do in our everyday lives — fly in an airplane, get treated at a hospital, or drive a vehicle, for example — come with inherent risks. The pilot could misinterpret a cockpit warning signal, the nurse could give the wrong dose of medication, or we could back our car into a fire hydrant. There are also global risks we didn't anticipate, such as the COVID-19 pandemic. And who knows what else lies ahead as our world becomes more complex?

But rather than labeling unexpected events as Black Swans or lying low because the gods must be crazy, there's a better way to see, understand, and interact with the world around us. We can manage our limited resources — governmental, organizational, and personal — to achieve optimal outcomes.

Predictive science suffers from an age-old dilemma: How do you prove you prevented the accident that didn't happen? There's a hidden pattern to how bad things unfold and a sequence to preventing them. In The Leader's Guide to Managing Risk: A Proven Method to Build Resilience and Reliability, Scott describes that science and shows you how to apply it in your business and everyday life.

The trick is to see, understand, and manage socio-technical risk — the everyday systems and human interactions in the world around us — in a specific order. In this book, Scott reveals the Sequence of Reliability and how he developed it. This secret to managing risk and preventing catastrophe is a simple, straightforward strategy that works for any endeavor — from crashes and pandemics to avoiding medical mistakes, to improving workforce morale, to achieving better results in our everyday lives.
